1. Completely fill out Section 1 of the application.
- A. Complete name, address, and telephone number.
- B. Your name
- C. Claim number or Payroll ID number
- D. Check savings or checking
- E. Member number with Max
- F. Check type of payment
- G. Only fill out if payment is an allotment
Once you fill out correct information, sign and date in Payee Certification Field.
2. Bring direct deposit sign-up form to the nearest branch or mail to the following address:
